Insurance & Payment Policy
What information do we collect?
We do NOT accept most insurance plans.
The only exception is:
UnitedHealthcare Community Plan for AHCCCS (Arizona Medicaid)
Important Information:
- We do not accept any commercial insurance plans (including Blue Cross, Aetna, Cigna, UnitedHealthcare PPO, Medicare, or any other major insurance).
- If you have UnitedHealthcare Community Plan AHCCCS, we do accept this plan. Please mention this when calling to schedule.
- For all other patients, we operate on a self-pay / cash-pay basis.
Why Many Patients Prefer Our Model
Because we are not restricted by insurance requirements, Dr. Hawkins can spend the necessary time with each patient to deliver thorough, personalized care focused on getting real results.
New Patient Visit:
$75 for a full 90-minute appointment
Includes: Detailed history, comprehensive exam, all necessary testing, and your first treatment
Follow-up Visits: $60 each
Many patients find this transparent pricing model more affordable in the long term and appreciate receiving focused care without unnecessary visits or restrictions common in insurance-based practices.
